1. We'll ask you to fill out our Restore Vision Checklist, so that we know that our treatment is not contraindicated with the conditions you are experiencing.
2. After our initial assessment is complete and favorable, we'll request your medical records so we can complete our detailed evaluation. 
3. Once we've determined that Fedorov Therapy can be of benefit, we'll give you a prognosis for vision improvement and help you get started on your journey to vision restoration.

1. Let us know the patient's medical condition. Tell us the diagnosis (if known), provide a brief description of your condition, indicating your main complaints. 
2. If we believe Fedorov Therapy can work for you, we’ll ask you to fill out our Restore Vision Checklist.
3. If our initial assessment is favorable, we’ll then request your medical records. 
4. Once we’ve determined that our treatment can be of benefit, we’ll provide you with a prognosis for vision improvement and a treatment plan.
